Our Patient Portal is a way for you to communicate online with your doctor and their office, view lab results and more.

Patient Portal is secure and confidential. The San Antonio Kidney Secure Patient Portal uses the latest technology to deliver secure communication between patients and their doctors. Your personal information will be treated with the same care and privacy as your medical records.

Patient Portal is not for an emergency. If you are experiencing a true emergency, you should call 911 or go immediately to your nearest hospital’s emergency room.

Create an Account from an Invitation

If you are looking to create your FollowMyHealth account from an invitation, please follow the steps below:

  1. Click on the registration link from your email. You will then see the following screens with the below options.
  2. Select the “Sign Up and Connect’ option.
  3. Next is the username and password screen. By default, the email address where your invitation was sent will be in the username field. It is recommended that you use your email address as your username. If you share an email address, or you want to use a different username, you can edit this field.

How do I Create a FollowMyHealth® Health Record Without an Invitation?

If you have not received an invitation e-mail, but wish to create an account, please see the following steps:

  1. Go to https://www.sakdc.followmyhealth.com (link will open in a new tab or window).
  2. Click the “I need to sign up” button (circled in red in the image below).
  3. Fill in the appropriate information seen in the next image below.  Then select “Confirm and Continue” at the bottom of the page.
    (Please provide the accurate zip code. This is the what we use to search for the local medical organization that connects to us.)
  4. Read through and “Accept the Terms” of Use by clicking on the “I Accept” button.
    Warning: if you select “I Decline” the account creation process will not complete.
  5. From here, you will create your FMH username and password.  For more details on this process, please go to this article.
  6. Once you have created a username, you will be taken to the “Request Connections to Healthcare Organizations” page. This can be searched by either zip code, provider name, or medical organization name.
    NOTE:  The search does default to the zip code that was input on the registration form in Step 3.
    Please be mindful that some medical organizations prefer that their patients connect via email invitation. You may get this invitation by contacting your provider’s office. If this is their preference, your medical organization may not appear in your search results.
  7. Click “Connect” next to the name of the desired medical organization (s). It will move from “Search Results” over to “My Organizations”. You may need to adjust the distance range on this screen. Once all your medical organizations are selected, click “Next” at the bottom right to continue.
    Note: In the event you hit “Skip This Step”, or if you have already created a FollowMyHealth account and are not connected to your medical organization, click here for instructions on how to request a connection from inside the patient portal.
  8. After hitting “Next”, you will need to “Accept Request for Access” for each of the organizations that you selected to connect with your portal.

After completing these steps, the medical organization will receive notice of your request for your medical records online. They will process your request and you will either receive notice of your connection or notice of the inability to match your account to your record, likely for security reasons. If this is the case, the e-mail should contain the next steps you need to complete the connection.

Creating an Account From an Invitation – Mobile

If you receive an invitation from your medical organization/practice, you can use your smartphone and/or tablet to create your account. It is similar to creating the account on the desktop. To create your account from an invitation on your device, follow these steps below.

  1. Click on the registration link in the email. Your invitation email may look different.
  2. Tap the option (First time using FollowMyHealth? Sign Up and Connect.)
  3.  Create your username and password. Note that your username will default to your email address. You can change this if you would like.
  4. You will then start the 5-step process to connect the account. You will want to accept the terms. Next will be the invite code. The code should be in your invitation email. Most codes are either the last 4 of your SSN or year of birth. If neither one works, contact your practice. Last, accept the release of information and your records will begin to load to your account.
  5. You will then get to a screen that will ask you to launch the application. If you have the FollowMyHealth™ app already installed, you can click on the icon. If not, click on the “Get It on Google Play” icon to download the free app. If you are using an iOS device, click on “Download” on the Apps Store.
  6. Once you download the app, open it and sign in.
